A lot of users are saying those are cold sores. Abreva could help but I know of better stuff you can use long term.

I have gotten cold sores for years and only when I started taking bht did they go away permanently. I used to take this BHT (Butylated Hydroxytoluene) 250mg, 500 capsules https://www.amazon.com/dp/B002KCC9LY/ref=cm_sw_r_cp_apa_i_OH5RDbQN8E9BN until I started buying my supplements in bulk.

BHT has cured me. I used to get cold sores not just on my lips, but on my chin and in my nose too!

Two other products that can significantly help are taking lysine and a zinc supplement. My favorite zinc supplement comes from Bluebonnet Nutrition. Zinc is best taken with food. Zinc gluconate in my opinion works a lot better than zinc citrate.

Going back to abreva, it used to work for me but then the cold sores I had adapted and the $20 tiny tube never worked again. If you've never used abreva then there's a high chance it'll work for you.

Cold sores have a tendency to recur in the same place and can spread. Make sure to sterilize or replace products that have touched the sores. Do not scratch them because the clear fluid that will ooze out will make it spread. You need the cold sores to dry out while healing-putting a band aid on it or Vaseline won't help.

So I would get the abreva, start taking zinc and/or BHT, and wait for everything to heal. I've had cold sores since I was little and I've tried everything prescription and non-prescription. BHT and zinc are the only two things to prevent and heal cold sores. PM if you want more info.

Lysine keeps me practically cold sore free as well. For your relative who suffers from shingles, you might try Butylated Hydroxytoluene. It's a food preservative that apparently helps against lipid-coated viruses (which also includes the virus that causes cold sores). It's not 'natural', but if it works, it works.
