Hey there! I've been in almost the exact same boat as you, with acne that does not respond to any conventional treatment at all. My skin is not great right now, but it's improved dramatically with my current routine. I'll start off by explaining that, then I'll give some general advice.

The biggest thing in my routine is to take 100mg of spironolactone twice a day. This was prescribed to me by a dermatologist. Because my acne is mostly hormonal, the things I put on my skin will never really help. Because this drug changes my hormonal makeup, it helps A LOT. Since your skin responded to birth control, I'm guessing that this is also the case for you.

For face wash, I use a Kaiser Permanente wash with 5% benzoyl peroxide. Now, my skin usually hates bp and it usually makes my acne much worse. However, using it in a face wash and letting it sit on my skin for a bit before rinsing it all off seems to give me some of the beneficial results with none of the irritation. I'm still experimenting with this a bit.

Next, I use Finacea gel, which is 15% azaleic acid. This is the only product that's ever given me major purging. Almost every other exfoliating product, from salicylic acid to tazorac, either gives me no results at all or irritates the hell out of my skin. This product does neither.

Lastly, I use Sebamed Care Gel. I've yet to find a moisturizer that does not break me out. All of them do, and it sucks. This is the closest I can get to a moisturizer that does not make my skin worse. It's very gentle and mostly consists of aloe.

For sunscreen, I use an innisfree product that you won't like because the brand is not cruelty-free (they sell their products in China). I'm still on the lookout for a new one.

As for general advice:

First, try eliminating as much as you can. I know that it's really, really hard to do but it's the only way to know if your products are making your skin worse or better. Try only rinsing your face with water for a month, if you can. Then introduce only a soap. Doing this is what made me realize that all moisturizers break me out.

Next, look for hormonal approaches. If you're against going on medication, you can try Evening Primrose Oil, which supposedly helps regulate hormones, but it had no effect on my acne.

My last bit of advice is to watch out for irritation, but accept flaking and peeling. If your acne is partially caused by your skin failing to shed dead skin cells properly, then there's going to be a nasty adjusting period where all those retained dead cells need to come off. The thing to watch out for is irritation. If you're flaking so much your skin is red and it stings to put products on, then you've exfoliated too much. But if that hasn't happened, then don't be scared off by flaking.

I tried various washes and lotions, but in the end what helped the most with my hormonal adult acne (dammit why doesnt is stop when you start getting grey hairs?), was to take an evening primrose oil, and a zinc tablet every day.
