Only this one so far, but I'm trying to find the time to work on my second. It's around 80% complete (followed by inevitable editing), but has taken me much longer than expected due to work and family occupying a very great deal of my time.
Check the index on Amazon, it covers pretty much what you have in your index, and I have found it to be a really enjoyable read.
Now, a free and open variant would of course be great, but definitely have a look at the book, no need to reinvent the wheel when you gather information (the kindle edition will set you back about $5).

Just a heads up, someone already did something like this and turned a pamphlet into a full book. It's called Dropping Acid - A Beginner's Guide to the Responsible Use of LSD for Self-Discovery. I bought it for myself and also for a friend as an introduction to acid.
