I have one kinda like that, it sucks. The thing always slips off and stops filling correctly. You can get a kit to replace all of it pretty cheap.
the flapper valve seat is the problem. You replaced the flapper, so it's fine, but something is keeping it from sealing.
drain the tank (turn the water off), dry off hte flapper seat and see if it's rough, or has something stuck to it. You can probably clean it with some soft scrub if it has mineral bbuild up.
If need be, replace the whole assemply. FluidMaster sells a $20 kit that basically replaces the entire toilet working parts
You might be better off just replacing everything.
the rust in your toilet is because the metal parts in the tank are corroding.. no amount of cleaning will help .. you need to get a rebuild kit for $20 .. one that has brass bolts for the tank

after you rebuild the internals in the tank you can use Lime-A-Way Liquid Toilet Bowl Cleaner it really works well .. put it on thick in the bowl and under the rim of the bowl before you go to sleep and it will be pretty much clean the next morning.. pretty amazing stuff.
Agreed, especially if there's more than one component giving you trouble. It's pretty simple to just replace them all for about $20 per toilet.
