Marie Sharp's Beware Comatose Hot Sauce. It's the best tasting, most flavorful truly HOT sauce I have ever tasted. I highly recommend it.

Don't believe the "comatose warning", it's not very hot compared to others. It's just a habanero carrot based sauce

I had never seen anyone talk about it which is why when I turned my bottle around one day I was surprised. You're right in that many of them seem to be in a normal range.

I don't know which ones I looked at in the grocery store, since most of them are some variation of habanero it's hard for me to remember but I was probably looking at some of their hotter ones. I must have just been unlucky with the ones I picked up, one was over 200mg, another above 100. I believe one of them was the smoked habanero which I still want to try.

It is odd though, Amazon shows that the Beware Comatose has 410mg just like the Belizean Heat while MyFitnessPal says only 12mg. Someone did ask how much sodium on Amazon and a couple people said 410mg here. I'm not sure if they are reading from their bottle or just going by the picture. If your bottle said 45mg though, I would trust that more than a picture on the internet.
