We had a similar problem and found that this one ( Brica Baby In-Sight Mirror, Gray https://www.amazon.com/dp/B004L2JJAK) works for us. It has several options including tether but as part of that it has a moldable wire and fabric adjustment for the angle.
A lightweight mirror can fix this. $12, it won't hurt the child in a crash even if it detaches, and you can see the child in the carseat. http://www.amazon.com/Brica-Baby-In-Sight-Mirror-Gray/dp/B004L2JJAK
