I went cold turkey with my first two kids (kid number 3 never used bottles). We did use the soft spout cups like these which may have made the switch easier.

Eventually, once he adjusted to that, I was able to switch him to a sippy that acts more like a regular cup, with a wide mouth and a hard ridge. He's fairly easily able to drink from a normal cup now.
