L3/L4 and some L4/L5 can affect bladder control. I'm hoping you didn't have any issues there. I didn't, but a friend had some issues and I felt really bad for him.

Nobody ever warns you about the pooping when they really should. "Take care of your back, or you might have painful poops, but not in the way you're thinking."

Good. I found that Pilates really helped me in recovery and during PT.

Also, the Multifidus Back Pain Solution is an interesting read. Lots of backing research and, while not a complete solution, it does seem to get results with muscle balance. It won't fix a physical obstruction like a disc herniation/rupture. Nothing will other than surgery or time.

If your doctor gives you the "go ahead" for BJJ, just be sure your instructor knows your restrictions. You'll probably not spaz as much as other newcomers since you know your body better than most of them at this point since back injuries really are whole-body injuries. Because of that, just pay attention to what your body is telling you during and, more importantly, after.

I had a fairly similar injury, that a stupidly left for years before getting fixed. Hence my hip muscles are incredibly tight no matter how much work I do on them, so its good you got the underlying issue sorted.

The 30 day toe touch challenge and the 90 day splits challenge in the side bar are probably good places to start, they should maintain/increase your ham string flexibility.

It actually only features one exercise (get on your hands and knees and extend one leg at time straight back), but it helped me get over 10+ years of back/sciatic problems
