If they are a CPP or do a lot of clinical stuff then Epocrates Essentials is SOOOO useful, albeit a bit expensive (still less expensive then the alternatives and a lot better IMO).
I know exactly how you feel. While I really enjoy the size of these pens, I write far too heavy with too much pressure to allow these guys to last more than a few days.
The Zebra F-402 series are solid metal with a rubberized grip on the end. The cap unscrews to replace the cartridge and it uses the same refills as the F-301--a win-win. You might have better luck with these.
If you wanna start to understand why, you can buy a Zebra F-701 for $5 at your local office supply store. That was my introduction to nice, metal manufactured pens.
What I really like to do is take an F-701, and buy an F-402 for $10. I replace the plastic mechanism in the F-701 with the metal mechanism from the F-402. Then I replace the branded clip on the 701 with the 402's blank clip. Now you have an all metal pen that looks nice with no branding on it, and it only cost you $15.

The Zebra F-701 and Zebra F-402 aren't exactly tactical, but they are pretty heavyweight and write really well. They take cheap refills too. I have the F-701 and the weight and sturdiness are great, however I don't really write that much, just come class notes and such.
It's a fairly hefty feeling pen. My personal favorite is the Zebra F-301. I also like the F-402. I think you can't go wrong with any of them, but the 701 is a weighty pen and a bit thick, but not so much you can't wear it in a breast pocket or waist pocket.
Zebra F-402 is fully stainless steel. I suppose if you were a bit more inclined you could sharpen the tip to make it even more penetrable but it's gonna make a nice sized hole even out of the package if necessary.

I'm planning on buying a TIHK for a friend thats going into vice. I think its always smart to have on you regardless if you are plain clothes or uniformed. Also a good pen like Zebra 402 or 701 is always a smart choice. They are nice enough to use but affordable enough to lose. Good hinged cuffs for off duty use and a low profile carrier is nice too. I don't know the laws and polices for officers in Queensland though.
