Some quick searching does find Ichiro Okouchi's name on the at least the first series of light novels (see here and here).

And even on the R2 Turn novels depending on where you look (here for instance).

But most places I see have Mamoru Iwasa as top billing: see here and here.

Even the wenku8 site lists Mamoru Iwasa as the author (here).

The point being, it looks like this may have largely been passed off to another author who had the liberty of filling in details as he saw fit.

Having said that omg does this thing read like a bad fan fiction. The final scene with Nunnally touching Lelouch's hand adds in a whole scene and dialogue where Lelouch grants Nunnally a Geass and asks her to never use it.

For the digital code, Right Stuf doesn't mention this coming with one while Amazon does. Funimation's site won't load for me to verify if it indeed comes with code, but whatever the product page says there should be the most accurate information.

I'm not sure about the new movie but regarding the cap movies, I just found them on amazon here. It said that they are dubbed.

Same. I don't know why Bandai doesn't branch it out a bit, though perhaps its a branding thing. It's not Gunpla if it's not based on a Gundam series or something.

Found pre-orders for the Metal Build Guren S.E.I.T.E.N. on Amazon, though I've seen a few other places too.

I had originally bought the Bandai DVDs for code geass in the US when they were first released but I don't believe US ever got an official Blu-ray release.

For my collection I ended up buying the blu-rays for each season from UK's amazon(not that badly priced even with Euro->dollar conversion) and have them shipped to the US. The problem with this is they won't easily play in US blu-ray players but I had means for getting around that.
