✔️ That seems to be the latest. It's still possible something could be messed up on your install. If a fresh image doesn't resolve the problem, you could try rolling back one version to LibreELEC-RPi2.arm-9.0.0.img.gz.

⚠️ That seems like a strong indication that the USB drive has something to do with it, but I can't imagine what. It has its own AC power adapter, so it probably isn't pulling voltage down when it spins up. If the drive isn't too full, would you consider reformatting it? I'd be interested to know if it behaves differently after formatting as a different file system, like ext4. You can go a step further and wipe out the whole partition table. I think LibreELEC should have the commands you need in order to format a drive from the terminal/ssh.
Dont think there's anything like that tbh.
Suggestions:
A standard TV hat case for the Pi4 with a USB3 JBOD enclosure.
For better cooling and performance but more expense consider fitting a Flirc case & ditching the TV hat for a USB tuner like the Happauge Dual-TV, allowing you to watch/record two channels simultaneously.

This cooling/case I have unplugged the fan, it's too loud. I'm looking for a quiet fan, but the heatsinks seem to have been adequate for now, I think.
My external drive is self-powered via AC, although it is not the best drive, it does work. I can access and play all of the media just fine, it's just that last stage of scanning the library where it all goes wrong. I forgot to mention that when you power back up, it has updated the library with the new media!
I'm currently moving a file over to /storage and I will try scanning library without the external drive plugged in to see if it makes a difference.

I did think about overvolting. It's something to explore if all else fails.
