I got myself a Crosstour 4k for recording my summer bikepacking trip. I also bought a kit of various attachments, mainly for the chest harness but also the monopod proved very useful, and a few spare batteries to keep it going for longer. (A battery will record between 1.5-2 hours.) Considering how cheap the camera is, I've considered getting a second one to mount under the saddle pointing backwards.
Apart from said bikepacking trip, I've tested it in London mounted on my handlebars. Mind you, I only used 720p @ 30 fps on normal quality to save disk space, so the picture is a bit porridgey. (The camera will do 1080p @ 30 fps, or 720p @ 60/30 fps, in fine/normal/low quality.)
For the price, I think it's a decent piece of kit. Interpret that as you wish. One thing to note, is that each time you remove the battery the time and date will reset. If you depend on that being overlayed you'll be setting it a lot, which I would personally find annoying, unless you always charge the battery in the camera. Other settings are persisted, the clock clearly has no backup power to keep it going.
I'm a fan of https://www.amazon.co.uk/dp/B00IPEAHVU/ref=pe_3187911_189395841_TE_dp_1 - it's cheap, waterproof, loads of pockets and a handy extra net folds out the bottom to pop your helmet in if needed.

That said, I've switched to a pannier back on a rack recently and really love it - it's much nicer to not have a sweaty back and it feels way more stable if you need to carry something heavy home, because the weight is much lower down.
I got this HP Odyssey backpack on the cheap through work last year: https://www.amazon.co.uk/HP-Odyssey-Sport-Backpack-Laptop/dp/B00VTPOH7M?th=1

It's been fantastic. Fit's plenty, keeps the weather out and is comfortable.
I use these one, Continental Touring Plus Reflex Bike Tire, but then i'm not racing or doing anything special with my bike other than commuting.
https://www.amazon.co.uk/dp/B001EIHYWE/ref=cm_sw_r_cp_api_MJ4RzbNBGM45A
interestingly though, my difference in speed between my road bike with skinny, semi slick tyres and my cyclocross bike with wider, grippier tyres is only 1km/hr. This is over 2,000km on my road and 8,500km on my cyclocross bike, primarily commuting . So very little in it. I do however hold more PR's proportionally with my road bike.
I use an old go pro. It's a hero 3+ silver, does 1080, great in daylight but not that good at night, it's a bit dark .
It's attached with a strap mount from Amazon. Not sure what would happen in a fall. Hopefully it would just fold up or the mount would pop open. You can always attach the camera to your handlebars but I prefer it looking where I do, also can get a lot of vibrations if it's mounted on the frame. https://www.amazon.co.uk/dp/B00ZCG168W/ref=cm_sw_r_cp_apa_i_vAy4DbM8K2V6B
Edit: I use the open back cover otherwise the sound won't get in, . Unless it's absolutely pouring with rain, then I use the full enclosure back.
https://www.amazon.co.uk/Crosstour-Action-Underwater-Control-Waterproof/dp/B07B3ZKDR7
I paid £40 for the first one, then £28 for the second on prime day. 32Gb micro SD cards are about £5.50 and the camera comes with the necessary mounting hardware and batteries. The battery life isn't great (a little over an hour at 1080p60) but that is good enough to get me to and from work.
Other generic models which could well be the same with a different badge are available for about the £30 mark I think.
I got this camera three weeks ago, and have used it every day since then. I don't know how it performs in low light, but it is perfect for me.
I bought these recently to go over normal trainers and they're great. I ride an electric bike and don't sweat much so in the rain I wear these and a waterproof motorcycle oversuit with all my normal clothes underneath.

I use this:

Helmet mount is here:
http://www.amazon.co.uk/Garmin-Vented-Helmet-Strap-Mount/dp/B00EPKGOMO/ref=pd_bxgy_ph_text_y
and you'll need a micro SD card, I got this one:
http://www.amazon.co.uk/gp/product/B00J2BSW9O?psc=1&redirect=true&ref_=oh_aui_detailpage_o01_s00
It's great, full 1080p, does a decent job at night. Fully waterproofed.
My friend (who rides through some rough parts of town) absolutely swears by these things https://www.amazon.co.uk/Mr-Tuffy-700x28-32-27x1-1-4-Inch/dp/B001CN6O8G/ref=sr_1_1?keywords=mr+tuffy&qid=1567598338&s=clothing&sr=8-1
I use a regular ol' 20L backpack - https://www.amazon.co.uk/gp/product/B07H87DL9C/ref=ppx_yo_dt_b_asin_title_o09_s00?ie=UTF8&psc=1 - and it fits all that into it easily. I think anything that's 20L and above would be fine.
will headtorch do the job?
https://www.amazon.co.uk/Fenix-HL55-Lumen-Year-warranty/dp/B00N40FV0W
Proper bright and excellent quality. Runs on single rechargeable and replaceable 18650 lithium cell.
If you prefer handlebar mounted one then check this out,
https://www.amazon.co.uk/Fenix-BC30-Light-Lumens-Metres/dp/B00LMX99TM
1800 lumen bad boy.
I bought this bell that you can mount on the brake handles. Recommend 100%:

The book Lost Lanes has some really nice rides. Somebody took it from my office, and I kinda miss it.
Try tyre sealant in the inner tubes?
I use a syringe to get it in there more easily.
Had the exact same question yesterday. After searching on here, I settled for the Finn and bought it today:
https://www.amazon.co.uk/Finn-Unisex-Adult-universal-holder/dp/B00D8Z9KPU
Edit: also that size of phone is on the smaller side these days :D
I got a cheap one of Amazon - footage is alright, but battery life is already slightly terrible. It last long enough for two commutes though so doing the job for now. I'll see how I find riding with it and maybe upgrade to proper GoPro down the line.
I really feel for you, but I think this is as a good a time as any to remind people to get a camera, use it and report bad drivers. I have two of these, and for £35 I may not be any safer, but if my family ever have to take a case against someone who murders me with their car they'll at least be closer to getting some kind of justice. The more geared up we all are, and the more we are willing to report dangerous behaviour on the roads, the safer all of us will be.
