This pad has been going strong for me for over 3 years:
http://www.amazon.com/PS2-Energy-Foam-Deluxe-Dance-Playstation/dp/B000GHG0BA
I was playing DDR on it for about 30 minutes 4-5 days a week. Nowadays I'm not playing on it, but it's a great pad for only a little over $50.
Only thing - the USB part of it breaks VERY easily if you pull the cable sideways while it's plugged in. I had to resolder the USB connection once, though it's continuing to work fine. If you just use a USB extension cable you shouldn't ever run into that problem.
For those who are curious, it's available on Amazon.
The 360 doesn't have a different button for every d-pad direction, and as a result I would frequently use items (or miss beats) accidentally all the time.
I ended up switching to this nice controller.
Of course, if you could make CotND stop accepting up + down as an alias for that item/spell/whatever, it would lower the stakes, but here we are.
