My ex girlfriend and I used to engage in wet play all the time, right on the bed. Never had any major problems. Usually it involved her on top and just completely releasing her entire bladder all over me. Sometimes she'd even stand up, holding onto the headboard, and let it all out. We did this probably at least once a week. At one point we stayed in bed for several hours while drinking heavily, and she did it 3-4 times in one night, resulting in a huuuuuuge wet mess. The mattress cover still held up pretty well.
The mattress pad does get weaker each time you wash it if you aren't careful, so at some point it is possible for a few drops to get through. If you're super concerned about it, use two! Or use towels in addition to it. You can't tell it's even there and can sleep on it regularly as well.
This item is great! Easy to wash and we have never had any leaks or anything. I wash they came in a larger size but we will probably end up just getting another to lay beside it. We just lay it down when we know pee stuff will happen, then toss it aside to wash it the next day when it is time to sleep.

We have this mattress protector. We keep it on the bed 24/7 (except when we wash and dry it), so we can wet the bed any time we want - which is AWESOME!
We hardly even notice it, really. The only thing is that it makes the bed a little bit warmer than normal. There is a slight crinkliness, but you get used to it really fast and we don't even notice at all.
We have only had a small leak once, and that's because a cat clawed a tiny corner once and we didn't notice. Aside from that it has been absolutely perfect, and we've had some really heavy piss play on it.
As far as care. Wash it like normal, but air dry it. Putting it through the dryer will ruin it.

I don't see why this sub can't be that place. It's not a sub for hosting professional porno, these are all real people doing the thing (and a shit ton of lurkers).
That said, to answer your questions:
a. Get some kind of waterproof barrier. It can be as sexy as this fitted, rugged, waterproof sheet, or as utilitarian as this regular mattress protector. Then get a cheap microfiber blanket or flannel sheet for absorbency. Or get the fancy Liberator Throw. More expensive, but a better all-in-one product.
b. All the positions. Go for distance. Upside down and backwards. Sixty-nining. Really whatever strikes your fancy... but from what I hear: use an enema before you literally pee up someone's butt.
As for places other than the bedroom... camping? idk, just don't fuck up an AirBnB
