This is what I got when I entered nursing school and still use it. After my class, my nursing school starting requiring this stethoscope. You can not go wrong with a Littman classic.

This is what I had through nursing school. under $100 and does a great job!
vanishing point is a fountain pen, a premium pen. It costs about $150 on the dot. Unlike other fountain pens this one has a click mechanism, so it doesn't dry out, and is great for charting.

A litmann classic 2 is going to be less than $100 and engraving shouldn't be much more. Litmann are pretty much the gold standard. These are heads above their competition, will last a lifetime surely through his entire medical school career, and more than likely residency. I suggest engraving because they are stolen often, if its engraved its his. The only better option i can think of is a cardiology one, which do cost more, but are overkill and only the cardiologist carry them.
Your choices are between Classic 2 and classic 3. If he doesn't have a stethoscope, this is it. This is what you get him, period. Engraving would be a HUGE plus.
