Alasdair Post-Quinn is generally considered the expert. If you aren't married to it being a book, Lucy Neatby has a class on Bluprint :)

I used a guide in the book Extreme Double Knitting. Basically you take your existing regular chart and invert the colors (A becomes B and B becomes A) and then you mirror it, so < would become >. Then you splice one column of the original chart, one of the new, one of the original, one of the new, and so on. It's tedious and makes my brain hurt a little but the results are so cool!

Edit: Here is an image that shows my modified chart side-by-side with the knitting. It's not terribly clear but it should give you some idea of what modding the chart looks like.

I referenced this book a lot when I did some practice swatches before I jumped on this big project, and it had some great CO and BO tips and techniques. I think I used the ones I found there. For CO I just did a long tail cast on using the opposite color to anchor each loop. For BO I pulled each stitch over the next two in the row, which gives a really nice set of v shapes in alternating colors.
