Koss have a great reputation for sound quality value for money. I have two classic pairs and they are great.

I use over-the-ear "ladies" sport headphones, and have to use the smallest bud cover -- the interchangeable rubbery part -- and actually smoosh the bud into my ear.
I use these headphones and they stay really well. And for carrying phones, try a hip belt like this. My arm band does well, but I use this when I have my keys and gels. It stays best when it's snug around your waist, so if you're conscious about that, it may not feel great, but it's another option.
I went through 4 pairs before finding these They're women specific, but fit me perfectly and stay in. They're cheap, the audio quality sucks compared to my headphones and speakers, but they stay in and they're apparently sweatproof. Works for me!
I love my Koss Fitclips. At least if we are talking about exercise headphones. $5 and way better than the average el cheapo buds.
