Patriot Tab is the way to go.

Very small, very fast, and cheaper than most of the other suggestions on this page.

I've always had great luck with Patriot memory. They do a small 3.0 thumbie for a reasonable price called the tab.

This is the 32gb version

USB 3.0, it isn't the smallest but it looks like a good carry.

Another one I just found:

Looks like it might do the slide out thing you're not sure about but it is REALLY small for USB 3.0 (they need to be big to keep up with speed) and consequently it suffers a little on speed but it perfectly meets all your other requirements.

32GB Patriot Tab

Now I've never actually used this one specifically, but I know that the Patriot brand is a reliable brand.
They make it in 32/16/8GB sizes. You can see the other sizes in the similar items.

The way I see it, if you're going to use up a USB port for more storage, it may as well be USB 3.0. I recommend for you one of the following:
