So Todd Phillips says "the ending scene was his first genuine laugh in the movie" (paraphrasing)

The Joker is a known unreliable narrator. When he spins his yarn and tells you his stories it's usually bullshit. So, this whole movie could in fact be bullshit. If it was all bullshit then the time he laughs in the asylum is the first genuine laugh because none of it was real. But the movie leaves it up for you to decide. It could be real. It could be not real. It's ambiguous.
