Are you taking a B12 supplement? I was taking a cyanocobalamin B12 and had some pretty bad breakouts. I switched to a methylcobalamin B12 and stopped taking it every day and it helped a lot.
It could also be something you're eating. If you've replaced meat and dairy with a lot of soy products (tofu and soymilk) then you could try eliminating soy for a bit to see if that helps.
Also, obviously keep your face clean. Wash twice a day with a glycerin based soap. I like Trader Joe's tea tree oil face wash.
Trader Joe's Tea Tree Oil Face Wash. It doesn't get rid of all the demons, but it's so much less irritating the benzoyl peroxide and salacilic acid and works very well. I have a spot treatment for cystic zits from the doctor and I moisturize with a mattifying moisturizer (I use Body Shop's Seaweed Mattifying Day Cream), since I have somewhat oily skin.
I had acne for a number of years. It's different for everyone, but when I stopped using harsh chemicals that nuked my face and started using an oil based cleanser with tea tree oil, I stopped getting all acne. It's magic. If you live near a Trader Joes, they sell it for $5 or so. Meijer and kroger in the Midwest sells a similar brand called Desert Essence. It smells weird and hurts if it gets in your eyes but it works. Bonus points if you get some jojoba oil from the same skincare line and use it after as a moisturizer. It seems counterintuitive to use oil to clean a broken out face but it seriously works. It changed my life. My skin usually adapts to cleansers after 6 months or so but this stuff still works after a year.
