Do you know what barbels are? If so, did it have them? If it didn't, it wasn't a koi. 12cm is not "relatively large" for a koi; it's about one third of their usual full size. If I remember right, koi should have 500 gallons per fish...so perhaps it was a koi that simply couldn't handle such a small pond. Still have to wonder if they weren't mistaken though.

I have two goldfish, both single-tails. One has never ever jumped, while the other is a skittish fellow who will try to flee the tank over simple things like someone startling him by approaching it too quickly - fortunately I have lids to keep him in! If you're monitoring the parameters of your pond and everything is good then I wouldn't worry, given the rarity of the incidences...but you should definitely keep track and keep your filter running at all times except during maintenance.
