I searched for it on amazon. It's a third party seller, but I got the one with the breathing tube. It was about 36 dollars with shipping, so it's on the cheaper side as far as GoPro accessories. Totally worth it, it's awesome!

I have 2 different mouth mounts but I purchased both as I'm not skilled enough to do it with a 3d printer. The first is by Ho Stevie and can be purchased on Amazon. Works well but if you are doing something that is slightly bouncy the camera will bounce. The other one I have is called MyGo made by "GoPro" (that's what it says on the Amazon page) and works even better because it has 2 plastic fins that reach back into the mouth peice so as you bite down you stabilize the camera better.
I use these mounts for surfing/water activities as they are both breathable. And obviously a leash is extra important here.
