This one has two HDMI inputs (also comes in a DisplayPort option), 3.5 mm audio in for Mic and out for headphone/speaker, and two USB for mouse and keyboard. I wonder if it would be possible to use a USB hub attached to one of the USB ports to allow yourself to use more than just a mouse and keyboard that can switch, but this should have what you're looking for, although with only two HDMI inputs, but it should do the trick I would assume.

You're looking for what's called a KVM (Keyboard Video Mouse) switch. They can get rather pricey depending on how many and what kind of ports you want.
