That's the Japanese limited edition, I'm pretty sure that's the only release that comes with a 4K disc. I got it off of amazon.co.jp, but it looks like none of the current sellers ship to the US. You could try a reshipping service, but I'd think something like ebay would be cheaper. There's also a US limited edition, but it doesn't have a 4K disc, and the packaging/extras are a bit different judging from this video.
