Yes. There are ones where the ball is permanent. Fairly common, actually. They sell adapters.

If you can't get that pipe off(mine wouldn't budge even using a huge pipe wrench) and don't want to tear a hole in the wall(mine had tile around it) you can use this: http://www.amazon.com/Whedon-American-Standard-Shower-Adapter/dp/B000KKPCXA I think there are 2 or 3 different sizes so double check the ball size.

You will remove the hand shower adapter. Lefty loose, righty tight. It may or may not detach itself before loosening out of the drop ear 90. In that case, a pair of pliers or needle nose pliers will work to unthread the nipple out of the drop ear 90.

Then replace that with that shower head nipple and you are good. Make sure to put teflon tape (in the direction of the thread) and pipe dope on the end going into the wall and threading into the drop ear 90.

I searched 'height adjustable shower head' on Amazon and found enough matching products. Something like this would probably work.

I am a fellow tall person, and this has been in my bathroom for two years. It's been a sanity saver!
