I ended up using this one, https://www.amazon.com/LoveRPi-Power-Over-Ethernet-PoE-Raspberry-Model/dp/B07WD7HXSQ/

I've been happy with it. I used an external 5V fan connected to the 3.3V pins. Originally I planned to build a circuit to control via PWM from the GPIO headers, but I've found temps and noise to be fine running off 3.3V.
