Something like this should do the job, male ends into your amp and computer headphone jacks and female end for your headphones.

Yes the headphones only have 4 pole 3.5mm connector. The combo, 4 pole, port can do the mic and audio over the one connector. Cellphones have combo ports and some laptops like my Lenovo Y700. It looks like ps4 and XBone also use it. Try the headpones on your cell and it should work with the mic and audio.
