That to me seems like the 4-pin fan header on the card could be going awry. Try unplugging it, put in a 4-pin to Molex and connect to your PSU. This will make the fan spin at 100%, but it's a good way to test if it something with the fans or the fan controller on the board.

Yes. In the very unlikely event you have trouble, you can get adapters that will take the PWM control input from the motherboard and combine it with voltage directly from the PSU like this or this to give you as many fans as your PSU will support (which is an unrealistically large number)

I found a solution without having to do any DIY:
