They used big-ass machines. One was the BBC standards converter. And, like all electronics these got smaller and rack-mounted and affordable.

This little converter from Amazon does what that whole machine does.

You actually need something to do an active conversion, so a box like this, for example.

NTSC has fewer lines of resolution, but a quicker framerate. PAL has more lines of resolution, but a slower framerate, so the timings for one are completely incompatible with the other. And there are differences in how the image is actually encoded in the signal. The result is that you need something to take in a frame of video, probably completely decode it into an image, then re-encode it into the other format.

I haven't actually used a converter that does that, but I imagine that it introduces a couple frames of lag, and probably doesn't make an image that covers the whole screen (I'd expect it to be squashed short maybe, especially in a cheap converter).

An AV-SCART box is usually just a cable adapter, without any conversion. SCART has pins for composite, RGB, S-Video, component, and audio, and its advantage is basically that you don't have to mess around with matching cable colors at the rear of the TV.

I suspect that the two best pictures you'll get would be connecting it to an HDTV (which should be able to handle both PAL and NTSC signals) or an NTSC CRT (powered through a 240->120V transformer, of course). That's mostly speculation. When I was in Europe, I had my US consoles, games, US TV, and transformers for the power requirements, so I never did anything with converters between NTSC and PAL.
