You did Insanity in your dining room?! Dang must be a big room haha. With Asylum you will need more space. If you have room in your garage, it would probably be the best place to do it.
I'm not familiar with those resistance bands but I did a quick check on it. Should be fine as long as you can fit them around your legs too.
These are the ones I have, but don't actually recommend: http://www.amazon.com/gp/aw/d/B0054MQUHQ/ref=mp_s_a_1_6?qid=1370459218&sr=8-6&pi=SL75
They are super tough to stretch, which I guess is actually a good thing. But the ones that the trainers use in the video look much easier and more elastic. You can get better, and cheaper ones at Target.
http://www.amazon.com/Timex-T5G971-Unisex-Personal-Monitor/dp/B000P8VWQS/ref=sr_1_2?ie=UTF8&qid=1342277278&sr=8-2&keywords=timex+heart+rate+monitor
Gives you the ability to set a lower and upper bounds on your heart rate with an audible chime if you're not within your zone. Also allows you to see your average heart rate, your calories burned, your total workout time, your total time in your heart range, and your peak heart rate.
Oh, and the time.
If you don't go with this model, make sure you get one where you can personally replace the battery yourself. The last thing you want to do is save $5-10 on a heart rate monitor but it costs you $20 to ship it back to the manufacturer to replace the battery.
I have been using this one: Oregon Scientific SE138 Strap-Free Heart Rate Monitor
I don't recommend it as it is reliant on finger contacts. It works well under ideal circumstances, but the middle of pure cardio doesn't seem to be an ideal circumstance. I may try out the one that Hiaskios recommended.
I went to a running store and they analysed my feet. I already knew that I have very, very flat feet and just like you, my feet were aching. I was not exhausted but my feet were in so much pain, that I could not keep going.
They set me up with these asics and I really like them (if you see that dark grey part, it's the arch support, and it's much harder than the rest of the sole), they have a crap ton of arch support. They hurt my feet a bit the first day, but have been fine today. I think my feet probably just need to get used to being supported.
Do you have a running shoe store that you can go to?
I have this heart rate monitor I bought at the end of month one, mostly out of curiosity to see how many calories I was burning and what my heart rate what at when I felt dead. I find it helps me push harder to keep 'digging deeper' :)
I bought some interlocking jigsaw mats that piece together.
http://www.amazon.com/gp/product/B003TUBQ0Q/ref=oh_details_o05_s00_i00
Each piece is 2ft x 2ft and each order comes with 4 pieces. I bought 3 sets for a total of 12 jigsaw pieces. I put this on top of my carpet and it works pretty well.
I found that with the high intensity workouts like Insanity, the jigsaw pieces would start to come apart. I had to put some duct tape on the bottom of the pieces to keep them sticking together. I also had to put a heavy weight at 2 of the corners to keep the mat from sliding. It was a mild annoyance, but I'm overall happy with the jigsaw mat. I think my 8ft x 6ft mat is a good size for me. A yoga mat is waaaayyyy too small for Insanity.
Your energy level will definitely rise as you go through Insanity.
I bought the Omron 100C last year when I tried to do p90x, it sat in a closet until the day before yesterday. Still works great.
Insanity is not a muscle gaining workout. You need to lift heavy weights to see real muscle growth. It will tone you but any sense of real muscle growth from insanity will probably come from losing the fat that's hiding it.
That being said, Insanity will improve your cardio which means, later on if you want to gain mass you will perform much better at the gym.
If you want to learn how to gain muscle I suggest reading Arnold's book.
Mats I purchased
I got 2 sets and it makes a 6' by 8' workout area that seems to be the perfect size for at least the first 2 workouts (I'm 6'4" for reference). Today will actually be my first day to use them.
I have this on hardwood floor. I also but a yoga mat in the middle for more cushion. The reviews with carpet liked it. http://www.amazon.com/Stamina-Folding-Equipment-84-Inch-36-Inch/dp/B0012MEZKG/ref=sr_1_5?ie=UTF8&qid=1367697773&sr=8-5&keywords=workout+mat
I'm about to jump back into Insanity this week for your same reasons (did it last summer, lost 29 lbs. and re-gained 15. I lost those 15 lbs with keto diet, but no muscle-building there on diet strictly.
The only supplements I used (and will use) were some Opti-Men tablets by Optimum Nutrition and Ripped Fuel fat burners. The latter has a good amount of caffeine, which can be beneficial if taken ~1 hr before your workout after a very light meal.
I also plan on taking protein supplement (Optimum Nutrition Double Rich Chocolate, or Trader Joe's vanilla whey for ~$15 is a steal), as I'll continue doing keto with Insanity.
You can get it on Amazon too.

While you do this program, try not to focus so much on losing weight, but rather losing FAT.
I suggest buying a scale that measures your body fat percentage (here's an example of one). Then, just record your body fat percentage every week, and THAT'S where you will start seeing results immediately. As it has been said here already, you are very likely to see increased results in your weight because muscle (which you will be packing in) is denser than fat. Here's a picture to illustrate this:
http://paleozonenutrition.com/wp-content/uploads/2012/01/fat-vs-muscle.jpg
Notice how much better she looks in the second picture, even if she is heavier. This is why YOU SHOULD NOT TRUST YOUR WEIGHT.
I usually wear Merrell Barefoot Pace Gloves. They are very lightweight and I find I can move quicker in them versus regular sneakers. She may want to wear a sneaker with more support every now and then. I find I get pain in my arches if I don't switch it up so.often. (I do insanity in my garage, FWIW.)
